Model of Diffuse Pulmonary Hemorrhage in Immature Rats

Abstract: 目的模拟临床肺血管严重充血的病因,建立弥漫性肺出血幼龄大鼠模型以期有助于对新生儿肺出血的研究.方法24只幼龄大鼠随机分为2组,每组12只.实验组皮下注射0.1%肾上腺素,每日2次,第一天剂量每次0.2 mg/kg,第2天每次0.3 mg/kg,以此逐渐增加至第7天0.8 mg/g.对照组以同样容量及方法注射生理盐水.实验组每死亡一只大鼠,对照组即随机处死一只大鼠.肉眼观察肺出血情况,将结果分为5级.Ⅰ级正常肺,Ⅱ级肺水肿,Ⅲ级点状肺出血,Ⅳ级局灶性肺出血,Ⅴ级弥漫性肺出血,并作光镜及电镜观察.结果平均用药天数5.24±1.82 d.肉眼观察实验组10只大鼠肺呈Ⅴ级改变,Ⅲ、Ⅳ级改变各一只,对照组12只大鼠肺均为Ⅰ级(RIDIT分析,x2=18.00,P<0.01).光镜见Ⅴ级肺有多处小叶融合性出血,出血区肺泡内充满红细胞,间质也出血,小血管及毛细血管重度扩张、瘀血.电镜示Ⅴ级肺肺泡Ⅰ型上皮局部崩解,气血屏障变薄及全层断连,肺泡腔内充满红细胞.结论采用肾上腺素皮下注射可成功制作严重肺血管充血所致弥漫性肺出血幼龄大鼠模型.
